Information for Patients

Before Admission

To ensure a smooth and prompt completion of the admission process, please bring the following documents and information with you upon your arrival at Cosmoclinic:

  • ID Card or Passport

  • Social Security Number (AMKA)

  • Tax Identification Number (AFM) and competent Tax Office (D.O.Y.)

  • Private insurance policy number, if applicable

  • Negative Molecular COVID-19 Test (PCR) performed within the last 48 hours

  • List of current medication

  • Any referring physician's instructions

  • Results from recent medical examinations

Patient Admission

The admission process for hospitalization at Cosmoclinic has been designed to be carried out in the most smooth and organized manner. Our goal is the prompt service of the patient and their relatives, alongside proper guidance from our staff, ensuring the process is completed without any inconvenience.

Upon arrival at the Clinic, the patient or their representative should proceed to the Admissions Office to provide the necessary documentation:

  • ID Card or Passport

  • Social Security Number (AMKA)

  • Tax Identification Number (AFM) and competent Tax Office (D.O.Y.)

  • Private insurance policy number, if applicable

  • Negative Molecular COVID-19 Test (PCR) performed within 48 hours

Once the patient's details are registered in the system, a nominal deposit is made, and the following forms are signed:

  • Patient Consent Form

  • Patient Information Guide

  • Declaration of Financial Responsibility

Following the completion of the admission process, the patient is escorted to their room for the necessary examinations to be conducted.

In case of an emergency admission during hours when our Secretariat is closed (10:30 p.m. – 6:30 a.m.), it is necessary to provide the required documents and sign the forms the following morning.

Patient Discharge

The day of discharge marks the completion of hospitalization and the patient's departure from the clinic.

Medical Clearance

  • Physician’s Approval: Discharge is granted following the decision of the attending physician, once the patient's health condition permits it.

  • Patient Briefing: The patient is informed about their health progress and receives instructions regarding the continuation of treatment, medication, and any follow-up appointment dates.

Discharge Form

A Discharge Summary is provided, containing details about the exit diagnosis and any treatment to be followed. Home care instructions are also provided, along with dates for health re-examinations, if deemed necessary by the attending physician.

Financial Settlement

Prior to departure, the review and settlement of financial outstanding balances take place at the Admissions Office. In the event that discharge occurs during hours when the Admissions Office is closed, a Debt Recognition Statement is signed by the patient or their representative, and payment is settled on the morning of the next business day.

Departure

  • After the procedures are completed, our staff accompanies the patient to the exit.

  • If necessary, assistance can be provided via wheelchair or transport by our ambulance.

Useful Information for Your Hospitalization

During your admission process, you will be required to sign the Patient Information Guide, which contains detailed information and regulations applicable at our Clinic.

Medical Briefing

Briefings by the attending physician are provided exclusively to individuals who have been authorized in writing by the patient. Authorized persons can receive updates on the patient's progress at the Doctors' Office (2nd floor) after 12:30 p.m.

Patient Companions

After visiting hours have ended, stay-overs are permitted for:

  • One relative, provided they have written permission from the Head Nurse. The "Companion Authorization Form" must include the details of both the patient and the relative, as well as their relationship. Visitors without this authorization are kindly requested to leave the nursing floors after visiting hours.

  • A professional caregiver (employed by the patient's family). The "Companion Authorization Form" must include the AMKA of both the caregiver and the employer. It is mandatory to provide the Head Nurse with a copy of the "ergosimo" (service coupon) for the employee's last month of employment.

Private Duty Nurses

If you require a private duty nurse, please contact the Head Nurse. Only nurses registered in the corresponding National Registry are allowed to stay in the patient rooms. Any private duty nurses found on clinic premises without the proper identification card and Registry Number will be asked to leave immediately.

Medication

During your hospitilization at Cosmoclinic, your attending physician is responsible for your medication. Medicine is administered by the nursing staff according to the physician’s instructions. During hospitalization, the consumption or administration of any other medication not provided by the nursing staff is strictly prohibited. Please inform your doctor about any medication you are already taking.

Meals

During your hospitilization, your diet is determined by your attending physician in collaboration with the Clinic’s Dietetic Department. Companions are requested not to provide, and patients not to consume, any outside food without the physician's approval. Cosmoclinic bears no responsibility for any health issues arising from the consumption of food not approved by the attending physician.

Personal Belongings and Valuables

Please do not bring or keep valuables or items of great value on the clinic premises. Cosmoclinic bears no responsibility for the loss of personal items or valuables belonging to patients, companions, or visitors, regardless of their value.

Financial Obligations

Please contact the Admissions Office, which operates Monday to Friday, 08:00 a.m. – 04:00 p.m. More information can be found under Financial Obligations.

Medical Record Copies

Cosmoclinic respects the privacy and rights of every patient and strictly follows current Data Protection (GDPR) legislation. To obtain copies of your medical records, the patient or their representative must complete the "Request for Medical Records" form at the Clinic’s Secretariat. A valid ID (ID card, passport, or driver’s license) must be presented. Copies are delivered within 10 business days in printed or electronic format. Cosmoclinic reserves the right to request additional documentation when required by law.

Patient Rights

Thank you for trusting Cosmoclinic with your health. According to Article 47 of Law 2071/1992, your rights as a patient are as follows:

  1. The patient has the right of access to hospital services most appropriate for the nature of their illness.

  2. The patient has the right to be provided with care with due respect for their human dignity. This care includes not only the general practice of medicine and nursing but also paramedical services, appropriate accommodation, proper treatment, and efficient administrative and technical service.

  3. The patient has the right to consent to or refuse any diagnostic or therapeutic act to be performed on them. In the case of a patient with partial or total mental incapacity, this right is exercised by the person legally acting on their behalf.

  4. The patient is entitled to request information regarding everything concerning their condition. The patient's interest is decisive and depends on the completeness and accuracy of the information provided. The information provided must allow the patient to form a full picture of the medical, social, and economic parameters of their condition and to make decisions themselves or participate in decision-making that may affect their subsequent life.

  5. The patient, or their representative in the case of paragraph 3, has the right to be fully informed in advance of the risks that may arise or result from the application of unusual or experimental diagnostic and therapeutic acts. The application of these acts on the patient takes place only after their specific consent. This consent may be revoked by the patient at any time. The patient must feel completely free in their decision to accept or reject any cooperation for the purposes of research or education. Their consent for any participation is their right and can be revoked at any time.

  6. The patient has the right, to the extent and under the actual conditions possible, to the protection of their private life. The confidential nature of information and the content of documents concerning them, the medical notes file, and findings must be guaranteed.

  7. The patient has the right to respect and recognition of their religious and ideological beliefs.

  8. The patient has the right to submit or file complaints and objections to the competent authorities and to receive full knowledge of the actions taken and the results thereof.

Furthermore, according to the General Data Protection Regulation (GDPR) [EU] 2016/679, as a Data Subject, you have the following rights:

  • Right of access to your data: You are entitled to gain access to the data we process, as well as to be provided with information regarding how your data is processed by our Clinic.

  • Right to rectification of your data in case of inaccuracy: You are entitled to correct or supplement your personal data if it is inaccurate or incomplete.

  • Right to erasure of your data in specific cases: You are entitled to request the erasure of some or all of your personal data in specific cases, provided there is no legal reason for us to continue processing it and on the condition that the interests of our Clinic are not affected.

  • Right to restriction of processing of your data: You may request that the processing of your personal data be restricted. In this case, we will have the right to store your personal data but not to process it further.

  • Right to object to the processing of your data: You may object to the further processing of your personal data for direct marketing purposes, as well as for scientific or historical research purposes, and for statistical purposes, for reasons related to your particular situation.

  • Right to data portability: You are entitled to obtain and reuse the personal data you have provided to our Clinic for your own purposes. You are entitled to receive an electronic copy of your personal data and to transfer it easily and securely to third parties without hindering its usability.

  • Right to lodge a complaint with the Data Protection Authority in the event of an unfortunate incident of data breach.

  • Right to withdraw your consent: You can easily withdraw, at any time and without consequences, the consent you have provided for the processing of your personal data for the provision of our medical services. Such withdrawal applies for the future and does not affect the lawfulness of the processing based on consent prior to its withdrawal.

Please note that the aforementioned rights are not absolute; there are specific conditions and limitations for their exercise. Cosmoclinic will inform you individually of any special exceptions or limitations, provided they apply to your case, following the submission of a relevant request.
